The game starts up with the title screen to the game that has Mickey under a light, with his back to the player. The text above him reads: "HAPPY MOUSE" with text below it saying, "MACHEN SIE FROH MICKEY" (MAKE MICKEY GLAD). If the player doesn’t press enter for a few minutes on the title screen, there is a chance that it will suddenly change to a distorted Mickey Mouse head with his eyes blacked out and his mouth opened wide with sharp teeth. This is accompanied by a loud, distorted scream. There is text under and above his head that read: "HAHAHAHAHAHAHAHAHAHAHAHA" Presumably mocking the player. After this, it would cut to a realistic Mickey head with his eyes bloodshot, and with human teeth. The text below him will read "KEEP MICKEY HAPPY".

In the game, you are playing as Mickey Mouse in a town like place. There is a sanity bar in the top left of the screen that is going down. Your objective is to collect Bowties to keep your sanity from going down, Avoid obstacles like the nooses and ‘water’ that make your sanity go down and get the goal. If your sanity reaches to about 80, Mickey’s face would turn to an upset expression and move slower, with the same piano banging sounds from the original Suicidemouse.avi video. If your sanity reaches to about 50, Mickey would grow a huge smile on his face, and the buildings in the background would go wobbly, and the background would just be static, with static playing in the background. If your sanity reaches to about 40, Mickey’s face would rot and have a much more creepy look, Mickey would move a lot faster this time, The buildings in the background would turn into creatures that would have multiple eyes and teeth. If your sanity gets to 20, Mickey would become all bloody, and what would look to be dead body’s scattered around the level. If your sanity reaches 0, The screen would cut to black and a gunshot sound would be heard. After this, It would cut to a Mickey Mouse head with Russian text below its head. Suddenly, the Mickey head and the background would have a red overlay to it, with the Mickey head having a wider smile, and having red pupils in his eyes. Suddenly, it would then show multiple images of a bloody, realistic 3D Mickey head with sharp teeth, and a green wireframe Mickey head. This is accompanied with a slowed down, loud, distorted scream. After this, it would take you back to the main menu.
