The playable portion of the screamer included instructions that were also narrated. Players were instructed to click on the flashlight every time one of the doll's eyes turned red to drive away Mary's ghost. The gameplay began immediately after the narration ended, even if the player had not clicked the button yet. The game displayed a darkened view of a shelf containing multiple ventriloquist dummies, and players used a flashlight to illuminate the area. Every so often, a pair of eyes would pop up over one of the dolls, and the player needed to click on them to make the eyes and the doll disappear (although the eyes might reappear in a place where they had already vanished). After the player clicked on their fifth pair of eyes, a close-up image of the ghost of Mary Shaw (portrayed by ''[[wikipedia:Judith Roberts (actress)|Judith Roberts]]'') appeared, shaking and screaming at the viewer.
