Creepy Ugly Guy is a name given to a disturbing, photoshopped image of a man, edited to have pigtails, a beard, a chimpanzee's eyes, and a smile, and having heterochromia. The image was popular in the early 2000s and was used in many screamers. The image is from the 90s game Kai's Power Goo, a game where you edit photos of people's faces. It is unknown when the image first appeared on the internet, or when it was first created.
The image was also featured in the music video of the song "Schfifty-Five", made by the band Group X[1]. According to Reddit user boogiemanster, they downloaded the image around 1997 on a website the user and his friend were making with Dreamweaver before it was obtained by Adobe. Again, according to YouTube user mixtersushi they found it from a parody website called "The Splice Girls" that parodied The Spice Girls.
