Fred.jpg was a screamer located on the website akk.li, where the famous Anne.jpg file was located on. The website is now defunct.
The Flash animation first shows the "Bitch please" meme (also known as Yao Ming's face) with an appearing text written in Spanish, which translates in English to "Once upon a time there was such an ugly man as that". However, as soon as the word "that" appears, the animation is interrupted by an image of a zombie with the same scream from What's Wrong?.
