The file name is bait-and-switch as it fools people into thinking it's a still JPG image and not a GIF. Ever since its creation (much like with other obscure screamers), it has been re-uploaded several times and copies of it can be found on various websites.

The image shows a grainy, black-and-white capture of a woman laughing in a dark room. A red circle is drawn around an orb of light that's right next to her head. About 4 seconds in, we then suddenly zoom in on her face as it becomes eyeless and a bit deformed. With this being a GIF image, there is no scream.

The GIF pretends to be a picture of a woman laughing and a red circle, supposedly leading the viewer to look at it; however, 4 seconds in, an eyeless and blurred image of her will zoom in, with no scream.
