The video starts showing a text stating the title's name, After a few seconds, the slideshow begins, with random Toy Story snapshots displaying, while You've Got a Friend in me by Randy Nevman blares in the background, the secret has nothing to do with the video, this goes on for 13 seconds, until a skull with red hair, wide mouth, spikey teeth and open tongue, pops up along with a loud scream.
The video starts with the title name and then proceeds to show a slideshow of random snapshots from [[wikipedia:Pixar|Pixar]]'s 1995 computer-animated film ''[[wikipedia:Toy Story|Toy Story]]'' and its [[wikipedia:Toy Story 2|sequel]], set to the tune of "[[Wikipedia:You've Got a Friend In Me|You've Got a Friend In Me]]" by [[Wikipedia:Randy Newman|Randy Newman]]. However, at the thirteen-second mark, an image of Fanged Freak (a mask created by the late [[wikipedia:Don Post|Don Post]]) suddenly appears on screen, accompanied by the [[Say It]] scream.


A reaction video was uploaded by user ricki leanne on August 23, 2008. It’s like a typical [[screamer]] reaction video, but with a [[screamer]] at the end. After some text appear after the reaction, the same skull from the original video pops up, but with another scream sound, then ends with text taunting the viewer for getting scared, this video tricks the unexpected viewer for being scared to watch the original but watch the reaction video instead.
A [[reaction video]] was uploaded on August 23rd, 2008, by user Ricki Leanne. It shows him using the screamer to prank his mother, father, and brother Richard (his sister, Lindsay, is left out as he had already shown the screamer to her). After the final text appears, the same skull from the original video appears with the same scream (but with high and distorted quality), and it ends with the narrator mocking the viewer for having startled.

Toy Story Secrets is a screamer video uploaded to YouTube on October 1st, 2006 by "nuralider". It is similar to Zelda subliminal Message.
