Stress Test was a Flash screamer game made by Liquid Generation on Mar 17th, 2006.

the game begins with a simple test or survey, if the player answers all of the questions, the game will calculate your answers, after you keep going through, it shows a loading screen, once it is done, a GIF of a man's head exploding from the movie Scanners pops up along with a loud scream, About 4 seconds after, a text will appear on your screen saying You Have been sabotaged, the screamer was taken down along with the entire Liquid Generation website.
