Fukouna Shoujo 03, Meaning "Unfortunate girl 03" in Japanese, is a shocking pixel art animation made by vvindowsme. It was posted on Pixiv in May 11, 2012.
The video/GIF shows a young girl wearing her blue swimsuit standing in a machine. The machine then shoots a laser, startling the girl. The girl ducks, trying to avoid the lasers, but is then picked up by force from the machine. The girl is then cut by the machine's lasers, removing her swimsuit in the process, and forcefully dragged out of her skin. The GIF then ends with her vomiting and dying.
The gif is, as the name implies, part of a series by the artist depicting “unfortunate girls” dying in horrific ways; however, none of the others have reached anywhere near the infamy of 03.
