It first shows a bridge with no cars, a highway under the bridge, and some buildings in the background. It says Look closely under the bridge. Do you see anything unusual? Obviously, there is nothing, but after a few seconds it shows a dog with his mouth opened, and with some man screaming. The picture then zooms in into his mouth. At 0:06 seconds, the word LOL is shown from left to right.
