As players progress through the game, each time they score a goal, something strange happens in the background:

Unlike other "Find the difference" screamers, the images here do actually have some differences. However, contrary to what the text at the beginning said, there are 4 differences, not 5.


*2 points: Strange yellow squares flash in the background.
*4 points: A creepy clown flashes briefly, accompanied by a laugh. The computer-controlled paddle palette becomes twice as large as normal.
*6 points: An edited image of a girl that might induce [[Trypophobia|trypophobia]] appears. The background and paddle turn red, and the ball becomes a skull.
*7 points: A red-filtered image of an eye with a mouth edited in as the pupil appears. The screen starts shaking, making it harder to play.
*9 points: A picture of a bloody severed mouth appears. Nothing else happens, but the computer-controlled paddle can now predict where the ball is going.

Scary Spot the difference - Medium is a screamer video made by Michael Robinson. The video has accumulated over 172,000 views since its upload to YouTube on June 4, 2007.

The video starts with a Windows Movie Maker intro stating the following: "There are 5 differences in the pictures. See if you can spot them all (Hint the 5th is in the bottom right, look closely)", Twoism by Boards of Canada plays as we are then shown two images of a black dog (the uploader's dog Dexter, as said in the video's description) wearing a blue hoodie.
